Yes, I have from Blue cross and blue shield, and I think the university pays about $ 3000 per year for me.

If you take full coverage (non deductible). In past I paid around $ 464 per month (minimum) for single person. Price will determine based on the pre-existing conditions, # of employees, state, and age of course.

I dont know in Nepal but in US I pay around 2400 a month for the full coverage(through the employer). What is the main purpose though...
